FORMS & RESOURCES

The current USFSA and WFSC memberships expire on June 30, 2026.  

You can renew online via Entryeeze or by filling out the membership packet which is linked below. We will try to have a few copies at the rink. You can also scan and email your application to winterhurstfsc@gmail.com but will still need to mail or deliver the check to the club. If you have any questions or can't renew online, please let us know via email: winterhurstfsc@gmail.com. 

 

There are multiple forms that require your signature: concussion,  Lindsay's Law, your respective "Code of Conduct",  WFSC Ice Rules, etc. You can sign them on Entryeeze or you can download, print, sign and bring them to the rink. 

Please Review all Skate Safe rules. The USFSA SkateSafe Program Information is available on the U.S. Figure Skating Website: www.usfigureskating.org. Click on SkateSafe on the home page. You will find the handbook and information on reporting, compliance and training.

 

If you are a Home Club member, the club automatically renews your US Figure Skating membership as soon as we receive your completed application and payment.
